Parent's or Guardian's declaration and consent
To the best of my knowledge, the information I have given is correct and the child named above has my permission to engage in all the camp’s activities including but not limited to canyoning, glacier expedition, ice climbing, ice skating, rock climbing, watersports, via ferrata, helicopter ride, ropes course, bridge swing, swimming and hiking unless otherwise noted and indicated in an enclosed written doctor’s report. I am aware that these activities have inherent risks and confirm the knowledge that these activities take place under the responsibility of the camp staff and also under the responsibility of companies or third persons carefully chosen and contracted by the camp.
I hereby give permission to the medical personnel selected by the camp to give treatment and for camp personnel to request treatment by a doctor for the child named above. In an emergency, if I cannot be contacted, I also give permission for the physician(s) selected by the camp to hospitalise, order X-rays, medications, anaesthesia, surgery or any other treatment considered necessary for the above named child and provide the camp personnel all medical information related to the treatment of my child, unless otherwise noted.
The camp is not responsible for loss of any personal property including money or other valuable items. The camp clearly states that it cannot be held responsible for extra costs, complications, losses, injuries, damage, accidents, lateness or inconvenience due to any error made by a company, or other persons in regard to the benefits offered in the reservation, during the activities or during transportation of participants unless some negligence or lack of diligence on the part of the camp can be proven.
The camp reserves the right to use photographs and movies of participants, or work completed by participants or other persons, and mandate, employ, hire or make use of staff and or professionals to direct pictures, audio and video recordings to use in publicity material and other extensions thereof; all works based upon, derived from, or incorporating this footage. These pictures, audio and videos including any modified versions (the ‘Footage’) are a proprietary product of the camp and is protected by Copyright law. The camp reserves all rights of ownership and copyright and all other intellectual property rights in any format or any medium of this footage. I acknowledge and agree that the Footage (and all rights therein, including, without limitation, copyrights) belongs to and shall be the sole and exclusive property of the camp and waive all moral rights in the performance in the Footage while the camp never publishes family names in any of the publications to safeguard privacy.
I have read and understood the contents of this Health and Consent Form and agree to all the policies stated therein.
